Child Health Surveillance
The Child Health Surveillance form covers child development from perinatal to pre-school stage.
- Either from the Summary Menu - Routine Checks - Child Health (or Alt-S, U, C)
- Or from the Medical Records Overview column on the Patient Record, click on the Child Health Surveillance line with the right mouse, and select either Summary Form or Management Plan. Both share similar entries.
- Or from the Medical Records Overview column on the Patient Record, click on the Child Health Surveillance line if it is coloured dark red, to show existing entries, and display these on the Filtered tab.
- Or from Select Guidelines, uncheck Central and tick Local - select CHS1 (6 week), CHS2 (8 month), CHS3 (18 month), CHS4 (3 year), CHS5 (4.5 years).
- select Add - Select Data Entry Form or click on . Double click on All Tabbed Forms or Clinical Data. Double click on Child Health Surveillance, or CHS Examinations (this lists the separate Add screens for all entries).
