Reasons, actions & outcomes


 

Standard reasons, actions and outcomes are pre-programmed into Pharmacy Manager as part of the base data. They provide an efficient way of building up an intervention, whilst allowing the recorder to add supplementary comments to ensure all relevant detail is retained.

 

Reasons

Reasons for an intervention will typically fall into one of these categories:

  • Dispensing Event - Prescription irregularity, legally incomplete, incomplete prescribed product details/dosage query with patient and/or prescriber, major interaction flagged by Pharmacy Manager, flagged interaction overridden after a clinical assessment, prescribed medication not wanted by patient, evidence of potential or actual interaction with co-prescribed OTC therapy

  • Lifestyle Advice - Advice given on anti-malarias, travel medicine, vitamin supplements, smoking cessation, weight management, increased exercise, alcohol consumption etc

  • Signposting - Direction of patient to other services or assistance, such as an out of hours service or NHS direct, a doctor following a blood pressure, cholesterol or other diagnostic test result

  • Support for Self-care - Self help measures to improve management of an existing or new condition

  • MUR - Intervention recording during an MUR appointment

  • Test Results - Intervention raised as a result of out of normal range measurements e.g. high cholesterol reading.

  • Enhanced Service - Locally arranged service with a recording requirement

Actions

Actions will often involve contacting the patient's GP  but often they will involve seeking clarification by consulting any source of guidance whether it be a hospital or a publication as the BNF, MIMs or even Pharmacy Manager's Knowledge Base.

Supplied actions include:

  • Prescriber contacted

  • Prescriber contacted: controlled drug

  • Practice manager/receptionist contacted

  • Patient consulted directly

  • Carer/representative consulted

  • Patient medication record

  • Action without GP contact

  • Consulted reference

  • Consulted suppler

  • Consulted information source

  • Consulted hospital

  • Consulted other healthcare worker or professional

  • Other

Outcomes

An intervention will often result in one of the following outcomes:

  • Prescription confirmed as written

  • Prescription clarified

  • Prescription changed in accordance with pharmacist's advice

  • Prescription changed by the prescriber

  • Prescription not dispensed - patient referred back to prescriber

  • Patient counselled

  • Patient referred to GP or other healthcare provider

  • MUR arranged

  • Other

You can also add your own reasons, actions and outcomes should the need arise. However, for this, you will need to input the System administration key which can be obtained from Service Desk.
