Vaccinations Service - Clinical Screening
The Vaccinations Service - Clinical Screening screen displays after selecting a vaccination type(s).
Complete the Clinical Screening sections for each type of vaccination selected:


This section displays the patient's previous NHS flu vaccination history when PDS is verified.
A warning displays if the patient has had a vaccination in the last six months.

Choose the vaccination type, select either:

NHS - To record an NHS Flu vaccination is being administered, and then select the Eligible Patient Group from the available list.

All children aged 2 or 3 years on 31 August 2025

Aged 65 years or over
-
Aged over 18 to under 65 in a clinical risk group - Select from the Clinical Risk Group list provided:

Chronic respiratory disease
-
Chronic heart disease
-
Chronic kidney disease
-
Chronic liver disease
-
Chronic neurological disease
-
Learning disability
-
Diabetes
-
Asplenia/Splenic dysfunction
-
Immunosuppression
-
Morbidly obese

Pregnant woman

Person in long-stay residential care home

Carer

Household contact of immunocompromised individuals

Employed through Direct Payment of Personal Health Budget

Health and social care staff (aged 18 and over)

Private - To record a private flu vaccination is being administered. A message displays advising 'By selecting this option you are confirming that you have signed a PGD (patient group direction) that permits you to administer private vaccination services'.
Note - Private vaccination details are not sent to the MYS portal or the patient's GP and are not updated in the patient's record.

Does the individual currently take any medications that may interact with the vaccine?

Has the individual had any vaccinations in the past 6 months?

Tick the appropriate response to each question and add any additional information when prompted:

Has the individual had a seasonal flu vaccination in the last 6 months?
Important - If the patient has had a flu vaccination in the last six months, a warning message displays advising 'Vaccine shouldn't be administered'. -
Has the individual ever had a confirmed anaphylactic allergic reaction to a previous dose of the influenza vaccine?

Does the individual have any relevant allergies?

Enter the details of the person who recorded the answers to the questions.

Choose the vaccination type, select either:

NHS - To record an NHS COVID-19 vaccination is being administered, and then select the Eligible Patient Group from the available list.

Adults aged 75 years and over
-
Residents in a care home for older adults
-
Individuals aged 6 months and over who are immunosuppressed
-

Private - To record a private COVID-19 vaccination is being administered. A message displays advising 'By selecting this option you are confirming that you have signed a PGD (patient group direction) that permits you to administer private vaccination services'.
Note - Private vaccination details are not sent to the MYS portal or the patient's GP and are not updated in the patient's record.

Has the individual experienced any serious adverse reaction after previous COVID-19 vaccine doses?

Has the individual indicated they are, or could be pregnant?

Does the individual have a history of anaphylaxis or significant allergic reactions to any vaccines or its ingredients?
Important - If you select Yes to any of the questions, a warning message displays advising 'A review of the responses by a clinician is required before proceeding with vaccination. Refer to Immunisation against infectious Diseases (The Green Book) Chapter 14a.'. -

Enter the details of the person who recorded the answers to the questions.

When complete, select either:
-
SAVE & EXIT
- To save the episode and exit to the Vaccinations Service screen. The episode is not completed and can be updated at a later date from the Vaccinations Service screen.

CONTINUE
- To move on to the Delivery screen.
