Chronic Disease Register - Add, List or Remove a Patient
- To enter a patient on a Chronic Disease Register:
Either select Add - Chronic Disease Registers (Alt-A-N) then one of the following:
- Asthma Register (see Asthma-Add, Asthma Register, Health Promotion)
- Hypertension Register (see Hypertension and Chronic Disease Register
- CHD Register (see CHD Chronic Disease Register)
- Stroke/TIA Register (see Stroke/TIA)
- Diabetes Register (see Diabetes entries and Chronic Disease Register)
- Diabetes Annual Check
- Epilepsy Register (see Neurological Disorders and Epilepsy).
Or from the navigation pane of the Patient Record, click on the Disease Registers line with the right mouse, and selecteither Summary or Management Plan. Point on the row level of the required disease; right click and select Add.
Or click on , selecting Clinical Entities - Chronic DiseaseRegisters. This lists the separate Add screens for all entries.
Or, first make an entry in Add - Medical History with an appropriate Read description and code, for example #H33 for asthma, F25 Epilepsy. When saving the history entry, you will be asked if you want to enter the patient on a Chronic Disease Register to monitor review. Answer Yes.
- When the Chronic Disease Register - Add screen displays, check the Read description. Enter a Recall date if required, any free text notes.
- You can record Health Promotion advice or intervention by clicking on on the Stroke/TIA, CHD or Hypertension screens.
- Finally click OK.
