Create a Personalised Care Plan Manager Register

Patients who are identified as being at risk of admissions should be added to the Personalised Care Plan Register. To do this you need to complete the Personalised Care Manager Register Template which is accessed from the Admissions Risk Stratification and Register Manager Reports.

Note - As you add the patient to the Personalised Care Manager Register, the report is automatically updated and the patient is moved to the appropriate list.
