Health Promotion Summary
The Health Promotion Summary screen summarises whether a record has been entered in each category, whether it is a qualifying record, and whether an intervention has been made in the last year.
You can reach the Health Promotion Summary:
- either from Summary - Health Promotion,
- or by clicking on from the toolbar.
- or from the Medical Records Overview column on the Patient Record, right mouse click on the HP Interventions line, and select eitherSummary FormorManagement Plan.
- or from the Alert form or Alerts section under the navigation pane, where missing data can be entered by clicking on the Health Promotion button.
Add a record by pointing the cursor directly at a line in any of the three columns, for example, point at a Smoking line, and press the right mouse button, then selecting Add. This displays an -Add screen for the category. As you OK each entry, the system will display the next one on the list. Records can also be viewed, listed.
Date Recorded - The left-hand column shows if a record has been made in a category and the date it was entered.
Red - If there is no record, the category title is in red
Blue - Once a record is entered, the category turns to blue and there is a tick in the adjacent box. The date is recorded in black on the line.
Categories - Smoking, BP, Adult Weight, Adult Height, Alcohol, Family History of CA/IHD.
Family History - Point to the line FH of CVA/IHD and right click, selecting Add. If there is no relevant family history, then on the Family History Add screen, place your cursor in Read Term of Family History and press the down arrow key until you reach 122..00 No Relevant family history. Leave Read Term of Condition blank. OK.
If there is relevant family history of CVA or IHD, place your cursor in Read Term of condition. Type either CVA or IHD and press Enter, then press F3 to select the term you want. You are NOT able to put Family History codes here so select, for example, G66 CVA unspecified, or G3 IHD -Ischaemic heart disease. Click OK.
Qualifying Record - The middle column on the Health Promotion Summary screen should show whether there is any record that qualifies for Health Promotion. (For a list of qualifying data in each band, see "Summary of Data for Bands 1-3, Asthma, Diabetes")
If there is no qualifying record, the box is blank, and the category title is in red.
If there is a qualifying record and the Health Promotion button has been used on that record, ie an intervention is offered, the qualifying record turns blue.
For example, a Hypertensive, CHD or Stroke/TIA qualifying record would mean the patient has been placed on the Hypertension, CHD or Stroke/TIA Registers respectively.
The categories are: Smoker, Hypertensive, CHD (Coronary Heart Disease), Stroke/TIA, Overweight/Obese, High Alcohol, Positive FH CVA/IHD
The Overweight/Obese entry title depends on the patient's recorded weight (ie BMI 25-29.9 for overweight, over 29.9 obese).
Note - On the Health Promotion Summary screen, concentrate on the left-hand and right-hand columns. Note that the middle section changes automatically according to the data recorded on the left.
Date Intervention Offered - The right-hand column shows the dates of records where an intervention has been entered for a qualifying record. Records for intervention (advice, treatment), are only valid within the last year. Once older than that, the record turns red on the Health Promotion Summary screen as a reminder (in the above example, an old intervention for Obese in 1993).
