Epilepsy Data Entries
- To enter an initial diagnosis of epilepsy, type #F25 in Read Term - Add and press Enter.
- On the History Add screen, complete the form and OK.
- You will be asked if you want to place this patient on the epilepsy register. Answer Yes if so. This places all your epileptic patients in one group, regardless of the Read description, and makes recalls easier (see Chronic Disease Management).
- The Epilepsy Register - Add screen displays. Record a recall if required. Click OK to accept.
Subsequent entries of epilepsy can be made on the Epilepsy Summary Form or Management Plan:
- Either right click on the Epilepsy line on the navigation pane (Medical Records Overview column) and select either Summary, Data Grid, or Management Plan. Click in the far right cell of the line to which you want to add data, or you may already have an Add-form at the bottom of the screen.
- Or select Summary - Disease Management - Epilepsy (or Alt-S, G, E). Right click against the entry you want to make and select Add.
Other entries:
Managed by - Read Term - 667 Epilepsy monitoring. In Managed by picklist, select from <none>, Hospital only, Community only, GP only, Shared. Other.
Last fit - Read Term - 6676 Last fit. Note that the Date of Last Fit will default to today's date and you may want to overwrite this date for accuracy. Optionally type in free text comments in Notes.
Fit details - Read Term - 6675 Fit frequency. Enter the number of fits in FitFrequency and qualify it with the picklist: <None>, Daily Weekly, Monthly, Quarterly, Yearly, > (greater than) Daily, > Weekly, > Monthly, > Quarterly, > Yearly, Less Frequent. In Asleep, the picklist has: <None>, Asleep, Awake, Asleep and awake. Tick Aura if present. Optionally type in free text comments in Notes.
DVLC informed - Tick the box DVLC Informed if this is so. Read Term - 6673 Driving licence.
