Unhealthy Behaviours QI (NHS Wales)
We have introduced the Unhealthy Behaviours QI (NHS Wales) pathway to assist you in monitoring unhealthy behaviours in both newly registered patients and patients with specific long-term or chronic conditions.
The unhealthy behaviours being monitored are:

Obesity / High BMI

High Risk Alcohol Intake
-
Tobacco Use
Patients with the following long-term or chronic conditions are being monitored:

Diabetes
-
Cardiovascular Disease:
-
Stroke
-
Heart Failure
-
Ischaemic Heart Disease
-
Atrial Fibrillation
-
-
Hypertension
-
Asthma
-
Chronic Obstructive Pulmonary Disease
The Unhealthy Behaviours QI (NHS Wales) pathway displays alerts for patients where monitoring is required.
