Accessing the Unhealthy Behaviours QI (NHS Wales) Clinical Template

To access the Unhealthy Behaviours QI (NHS Wales) Clinical Template:

1. From Consultation Manager, select the patient required.

2. From the Vision+ floating toolbar , select Clinical Templates and then select Unhealthy Behaviours QI (NHS Wales):

   

3. The Unhealthy Behaviours QI (NHS Wales) template displays:

   

   The template is split into six tabs:

   • History - Displays clinical information from the patient's record.

     See Viewing the History Tab for details.

   • Identification - This tab details the reason the patient has been identified, either they are a newly registered patient or they are attending clinics or chronic condition reviews for a long term condition. Complete as required.

   • BMI - Use the BMI tab to record a height, weight and body mass index for the patient. If required you can also record one or more weight management interventions. Complete as required.

   • Alcohol - Use the Alcohol tab to record the number of alcohol units consumed per week by the patient. If required you can record one or more alcohol interventions. Complete as required.

   • Smoking - Use the Smoking tab to record the current smoking status for the patient. If required you can record one or more smoking cessation interventions. Complete as required.

   • Guidance - Provides additional information about Unhealthy Behaviours.

4. Select Save & Close to save the data to the patient's record.
