Access Types

The following are selectable access types and their description:

Access Type Description

All Users Roles Removed from Service

An administrator has removed all users from a practice or service

Announcement with Patient An announcement has been created, viewed or recalled in Tasks

Application Launched

An application has been launched in the portal

Authorisation Failed - the service user account is invalid

A user has attempted to login but has no business activities and therefore has been refused

Authorisation Granted

User has successfully logged into the Portal

Clinical data extract done

The system has successfully generated a clinical data report

Configuration Changed

User has changed their settings in the portal

Create legitimate relationship

A portal user has requested and been granted a legitimate relationship with a patient

Data extract result done

The Extract Reporting system has successfully generated a clinical data report

Data model auto-upgrade successful

Vision 3 system software has been updated

Download Audit Logs

An auditor has exported audit logs to their PC

GPAD version change

A Vision 3 GP Adapter version has changed

Logged Out

User has logged out of the Portal

Login

User has successfully logged into the Audit application

Login Failed - Exceeded maximum number of allowed failed login attempts, account locked

User has attempted login with the wrong password three times and is now locked out

Login Failed - Incorrect Password. 1 try remaining.

User has attempted login with the wrong password twice

Login Failed - Incorrect Password. 2 tries remaining.

User has attempted login with the wrong password once

Login Failed - Unrecognised username

Username entered is not recognised

Login Failed - no valid service user record or activities for principal

The is an internal system user and is not permitted to login to the portal

Login Failed - the user account is currently inactive

User has attempted to login but their account is set to inactive and therefore have been refused

Login Failed - the user account is currently locked

User has attempted to login but their account is locked and therefore have been refused

Patient Record - Create Consultation

User has recorded a clinical record for a patient using Practice Access

Patient Record - Create Record

User has started a consultation with a patient using Practice Access

Patient Record - Delete Record

User has removed a clinical record for a patient using Practice Access

Patient Record - Edit Consultation

User has edited a consultation record for a patient using Practice Access

Patient Record - Edit Record

User has edited a clinical record for a patient using Practice Access

Patient Record - View Data Failed

User has attempted to view a patient's record but has been refused

Patient Record - View Record

User has recorded a clinical record for a patient using Practice Access

Patient Search

User has searched for a patient

Patient is in a customer situation

Patient has more than one permanent record

Patient is in a customer situation - registered at more than two practices

Patient has more than one permanent record at two or more practices

Patient selected

User has selected a patient from the patient search results in the portal

Patient unselected

User has deselected a patient from the patient search results in the portal

Practice Access / no decision support acceptance

User has read and acknowledged the lack of decision support statement in Practice Access

Prescription - Printed

User has printed a prescription

Reverting to previous, implicit Sharing Opt In/Out

Deprecated event - ignore

Sensitive Task A sensitive task has been either created, Edited, viewed, rejected, deleted, completed or forwarded with our without LR
Sensitive Task with Patient A sensitive task has been either created, Edited, viewed, rejected, deleted, completed or forwarded with our without LR

Service Activated

Administrator has set a practice or service to active

Service Agreement Created (Sharing)

Administrator has enabled clinical data to be written from a Federated service back to a practice

Service Agreement Created (Viewing)

Administrator has enabled clinical data to be viewed from a practice by a Federated service

Service Agreement Removed (Sharing)

Administrator has disabled clinical data from being written from a Federated service back to a practice

Service Agreement Removed (Viewing)

Administrator has disabled clinical data from being viewed from a practice by a Federated service

Service Applications Updated

Administrator has updated the applications assigned to a practice or service

Service Contact Details Updated

Administrator has updated the address or telephone number of a service

Service Created

Administrator has created a new practice or service

Service Details Updated

Administrator has updated a practice or service details

Service Inactivated

Administrator has set a practice or service to inactive

Service Message Feed Updated

Administrator has updated a practice message feeds details

Set Sharing Opt In by Read Code

Deprecated event - ignore

Set Sharing Opt Out by Read Code

Deprecated event - ignore

Sharing Agreement Created

Administrator has created a new data sharing agreement

Sharing Agreement Replaced

Administrator has replaced an existing sharing agreement with a new data sharing agreement

Task with Patient A sensitive task has been either created, Edited, viewed, rejected, deleted, completed or forwarded with our without LR

Transferred Out

A patient has transferred out of a practice

Transferred Out (Adopted Child)

A patient has transferred out of a practice because of Adopted child

Transferred Out (Correspondence states 'gone away')
