The Patient's MUR Form
The changes to MUR introduced in July 2012 removed the dependency for an official MUR form layout.
As a result, Cegedim have devised a new copy of the full MUR form for each patient, to reflect the data collected from each MUR for summarised central reporting.
There is no longer a requirement to inform the patient's GP that an MUR has taken place. Where required there is a GP summary that can be produced and forwarded to the patient's GP.
Subsequent pages cover the reviewed medication history with an indication of any matters arising that have come out of the review.
If any the medication items that were reviewed identified no concerns, they are separately listed underneath, importantly indicating that they were part of the review process.
Depending
on how may items have been reviewed, the printed report may cover more
than two pages.
