How to apply the eFI Stratification Tool

Practices in England are required to identify patients over 65 with moderate or severe frailty. Some of the reporting lines in Vision+ Practice Reports, categorise patients who have an estimated risk of moderate or severe frailty but who do not have a score recorded. Below describes how to add the score to an individual patient.

Applying the score to an individual patient

  1. Highlight the line "No Frailty score recorded, Estimated as severe frailty" and select Expand .
  2. Highlight the patient and select electronic Frailty Index from the Toolbar.
  3. From the calculator screen click OK or OK & Attach to record the score.
  4. To record the severity clinical term (as part of the GMS contract), from the floating toolbar select Codeset .
    Note - Show Template also allows you to record the severity clinical term from the eFI template screen.
  5. Alternatively, if you have reviewed the patient you can do this on a patient by patient basis in Consultation Manager.
