Add the Patient to the Personalised Care Manager Register

You must also add the patient to the Admissions Risk Register.

1. Select the Codeset icon on the Admissions Risk Register - Add line:

   

2. Place a tick in the Apply box alongside the clinical term 8CV4 - Admission Avoidance care started, optionally change the date.
3. Select OK.
