Health Promotion Entry and Intervention

Record a Health Promotion entry and intervention

You can record a Health Promotion :

  1. either from individual Add screens, such as Smoking, where there is an HP icon
  2. from Add - Health Promotion Interventions - this just records that you have given the patient advice, a leaflet or some other "intervention".
  3. or from HP Interventions Management Plan - similarly, this just records that you have given the patient an HP intervention.
  4. or from the Health Promotion Summary

(1) Using on screens, for example, smoking, BP

After having entered data for, say, BP, Smoking, Alcohol, you may want to record an intervention - eg, for Blood Pressure records, this might be the start of therapy treatment; or for an overweight patient, advice on diet or an invitation to attend a Weight Loss Clinic.

  1. On Add screens such as Smoking, Blood Pressure, Alcohol, Weight have a Health Promotion button , first record the data relevant for Health Promotion, for example, the weight, the BP reading.
  2. Click on the HP icon. Today's date is recorded as the Date Intervention Offered and Clinician Offering Intervention defaults to the GP signed on.
  3. Optionally add extra notes by clicking on the Notes icon.
  4. Click OK.
  5. The apple icon will now appear ticked
  6. The READ code used will be 6781 Health education offered, with the exception of Smoking HP which enters a code of 8CAL Smoking Cessation Advice which is acceptable for the GMS contract.

(2) Add - Health Promotion Interventions

  1. Select Add - Health Promotion Interventions.
  2. From the menu, select the relevant Health Promotion Intervention screen:
  3. Smoking
  4. Hypertension
  5. Coronary Heart Disease
  6. Stroke/TIA
  7. Overweight on Weight/Height or Physiological Measurements
  8. High Alcohol on Alcohol - Add
  9. FH of Cardiovascular Disease
  10. You can record that you have made an "intervention" (leaflet, advice, clinic invitation). There are only two fields - Date InterventionOffered (which defaults to today's date) and Clinician Offering Intervention (defaults to the GP signed on).
  11. Optionally add extra notes by clicking on the Notes icon.
  12. Click OK.

(3) HP Interventions Management Plan

The HP Interventions Management Plan can be reached by a right mouse click on the HP Interventions line on the Medical Records Column of Patient Record; then select either Summary Form or Management Plan.

This includes fields for Health Promotion - Smoking, Hypertension, Overweight, Alcohol, CHD, Stroke/TIA, and FH of CVA/IHD and a final row Advice Given.

Right mouse click on the row where you want to record, select Add and complete the entry. An apple icon records the HP Intervention.

Use the last row Advice Given to record advice outside the other HP rows, such as breast examination. If a Patient Information Leaflet has been printed and given to the patient, this is recorded on the Advice Given row with a speak-bubble icon .

Chronic Disease Management Register

Placing a patient on the CHD, Hypertension or Stroke/TIA register will include their records for the Health Promotion Banding Reports, if they fall within the age group of the target population for Health Promotion (15-74). If relevant, from Add - Chronic Disease Registers, make an entry in one of the Registers (see ):

Asthma, Hypertension, CHD, Stroke/TIA, Diabetes

Family History

Family History of CVA/IHD can be recorded from the Health Promotion Summary screen by pointing at the FH of CVA/IHD line (left or middle columns), right click, and select Add.

On the Family History Add screen, if there is relevant family history of CVA or IHD, place your cursor in Read Term of condition. Type either CVA or IHD and press Enter., then press F3 to select the term you want. You are NOT able to put Family History codes here so select, for example, G66 CVA unspecified, or G3 IHD -Ischaemic heart disease. Click OK.

If there is no relevant family history, then on the Family History Add screen, place your cursor in Read Term of Family History and press the down arrow key until you reach 122..00 No Relevant family history. Leave Read Term of Condition blank.

You will then see a screen - Select Family History Code with the following choices:

  1. For CVA
    • 1225.11 No FH: CVA/Stroke/TIA
    • 12C4.11 FH: CVA
  2. For IHD:
    • 1226.00 No FH: Ischaemic heart disease
    • 12C2.00 FH: Ischaemic heart dis. <60
    • 12C3.00 FH: Ischaemic heart dis. >60
    • 12C5.12 FH: Ischaemic heart disease

Select one of these as appropriate.
