Diabetes Entries and Chronic Disease Register

You may make an entry for diabetes in:

  • Add - Medical History, or Read Term- Add, for example, using #C10 in Read Term for an initial diagnosis of Diabetes mellitus (see Diabetes entry by Medical History). You will be asked next if you want to place the patient on the diabetic register.
  • Add -Chronic Disease Management to place a patient on the diabetic register directly.
  • Diabetes Guideline from the Guideline Index (see Endocrine Guidelines);
  • From the navigation pane of the Patient Record, right click on the Diabetes line and select either Summary, Data Grid or Management Plan (see Diabetes Management Plan and Diabetes Summary Form. The Diabetes guideline can also be accessed from the Guideline Index.
  • From the navigation pane of the Patient Record, click on the Diabetes line to show the entries chronologically under the Filtered tab. Right click then Add.

Any initial entry of diabetes prompts the system to ask you if you want to place the patient on the diabetes chronic disease register. This places all your diabetic patients in one group, regardless of the Read description, and makes recalls easier (see "Chronic Disease Management").

The chronic disease register for diabetes also includes a second option - the Diabetes Annual Check.
