Consultation

• Asthma Consultation - Select the GP in Seen by. Select from the Read term picklist is: 66YK Asthma follow-up (default), 663..11 Asthma monitoring, 66YJ Asthma annual review, 66Yp Asthma review using Royal College of Physicians three questions, 9OJA Asthma monitoring check.
• Asthma Concerns - There is no default Read term for this screen and entry of one is optional.. Instead, or as well as, you may enter free text notes.
• Smoking - see Smoking
• Inhaler Ability - Select from Good, Moderate, Poor or Not examined. The default Read code is 6637 Inhaler technique observed. Other options are 6636 Inhaler technique shown, 663H Inhaler technique good, 663I Inhaler technique - poor, 66Y4 Inhaler technique - moderate.
• Night Cough - Select from No night cough, , 1week, 103/week, . 3/week. Read code default: 1717 Night cough present. The other option is 1718 Night cough absent.
• Asthma night time symptoms:

• 663O Asthma not disturbing sleep (Read code default)
• 663N Asthma disturbing sleep

• Asthma Daytime symptoms:

• 663q Asthma daytime symptoms (Read code default)
• 663s Asthma never causes daytime symptoms

  Qualify the record with a number in Frequency together with selection from the picklist: Daily, Weekly, Monthly, Quarterly, Yearly, >Daily, >Weekly, >Monthly, >Quarterly, >Yearly, Less frequent

• Asthma limiting activity

• 663Q Asthma not limiting activities (Read code default)
• 663P Asthma limiting activities

• Time off work in last 3 months - Select the period of time off from the picklist.The Read default is 13J Employment milestones, and the other option is 13JX Time off work.
• Asthma Last Attack - choose one or more of the following: Oral steroids, Nebulised, and Hospitalised. Optionally, enter free text Notes. The default is H33 Asthma.
• Height
• Chest X-ray - Change the Result Qualifier to Normal or Abnormal, and optionally add any free text in Notes. The default Read is 535 Standard chest X-ray.
