Diabetes Summary Form

The Diabetes Management Plan and Diabetes Summary Form group together all the likely entries needed for managing an asthmatic patient. The entries are more detailed on the Diabetes Management Plan which is system-distributed guideline, some parts of which are displayed in grid format, a column for each consultation.

Display the Diabetes Summary Form by one of the following:

  • From the navigation pane of the Patient Record, right mouse click on the Diabetes line and select either Summary - this displays a list of data entries. Note that if your Patient Record view does not have a Summary/Data Grid tab, you will only have the right click option of Management Plan.
  • From the Summary Menu - Disease Management - Diabetes (or Alt-S, G, D) - this displays a tabbed form.
  • Or from Options on Navigation pane (Consultation View), select Management - Disease Management - Diabetes.

The entries on the Summary Form include:

  • Diabetic register
  • Diabetes annual check
  • Current diabetes status
  • Diabetic consultation
  • Diabetic Retinopathy screening
  • Diabetes concerns
  • Insulin dosage comment

The Diabetes Summary Form has three sections: Management, Examination and Investigations. Click on each tab in turn.

To make an entry, point the cursor at an entry, for example, point to Diabetic Surveillance, click on the right mouse button, then click on Add. The relevant Add screen displays and you can complete each screen in turn, or skip to subsequent screens, or eventually press Cancel if you do not want to make an entry at a screen.
