Stroke/TIA
You can use Medical History to record a diagnosis of stroke. Any entry of stroke/TIA will prompt the system to ask you if you want to place the patient on the Stroke/TIA chronic disease register. This places all your stroke patients in one group, regardless of the Read description, and makes recalls easier.
- First select a patient and start a consultation.
- Either select Add - Medical History from the menu; or from Patients Records classic view, click on the green line Record at the top left under the red Alert line; or click on Options (MRO menu) then Diagnosis.
- At the History or Read Term-Add screen, make an entry for Stroke/TIA in the Read description, for example, by entering a keyword of stroke and pressing F3 to display a list of options.
If the message No Match displays, you may not have stroke in your practice Formulary, if you have Display from Formulary checked).
- If you click on G61.. Stroke due to intracerebral haemorrhage, then on Display Preferred term, *Intracerebral haemorrhage displays. Double click on this if you want to accept this term. Then if at the Read Term-Add screen, click OK to reach History Add. For other Read terms.
- Complete the entries on CV/BP Consultation Add screen.
- When you click on OK, you will be asked if you want to add this patient to the Stroke Chronic Disease Register.
- Answer Yes if you do and to display the Chronic Disease Register - Stroke / TIA - Add screen. For further help, refer to Chronic Disease Management.
- You can also place the patient on the register directly at any time from Add - Chronic Disease Management - Stroke/TIA Register.
- Health Promotion - Click on to record a Health Promotion (see Health Promotion).
- Click on OK.
Note - You can also record Health Promotion interventions for stroke patients either from the HP Interventions line on the Navigation pane of the Patient Record - right click and choose either Summary Form or Management Plan; or from the Add menu, selecting Health Promotion Intervention - Stroke/TIA.
