Merge patients

It sometimes happens that "duplicate" patient records occur. This happens when a name is misspelt; or one record might have one initial and another two.

Examples:

     - There are duplicate records for Catherine Jones, Kate Jones and Cathy Jones, depending on the prescriber!

OR

     - A woman who has recently married, and is on the system under two names;

Merging Patients is the way to merge these duplicate records.
