PDS Confused Local Records

When an NHS number is being used by two or more patients their clinical records become confused over time. It is normally the result of the incorrect patient being selected when tracing, probably due to similar demographic information.

Once found, the details must be safely separated in to two distinct records.

In the very rare instance that this happens on your system, the following warning message appears when accessing the patient record:

The record is now decoupled from the Personal Demographics Service (PDS) and requires local resolution prior to re-coupling (if appropriate).
