The transfer of the Medical Record Envelope (MRE) continues for the following reasons:

• The use of Electronic Health Records (EHR) for direct patient care is not across 100% of patients

• A large percentage of patient information from outside of practices remains paper-based

• The variable degree to which such external information is incorporated into the EHR.

Therefore, the need for a patient summary of the patient's EHR to accompany the MRE continues.
