Summary of Health Promotion Band 1

  • Target Population: Aged between 15 and 74 years on report date
  • No. of non-smokers: Patients with last smoking record of Never Smoked or Ex-Smoker on Smoking (Add)
  • No. of smokers: Patients with last smoking record of Smoker on Smoking (Add)
  • Smokers offered Health Promotion: Smokers with Smoker entered on HP Interventions (Add) within the last year

Summary of Health Promotion Band 2

  • Target Population: Aged between 15 and 74 years on report date
  • With BP recorded: Patients with any record of blood pressure in specified report period
  • With raised BP: Patients with any record of Hypertension Register.
  • Intervention for raised BP: Patients with raised BP (above) and intervention for High BP or Hypertension in HP Interventions (Add) within the last year
  • With CHD: Patients with any record on CHD Register
  • Intervention for CHD: Patients with CHD (above), and CHD in HP Interventions within the last year
  • With Stroke/TIA: Patients with any record on Stroke Register
  • Intervention for Stroke/TIA: Patients with Stroke/TIA (above), and Stroke/TIA in HP Interventions within the last year

Summary of Health Promotion Band 3

  • Family History entries can be recorded from the Family History-Add screen, reaching by clicking on , select Clinical Data, then Family History.
  • Target Population: Aged between 15 and 74 years on report date
  • With BMI recorded: Patients with any record of height and any record of weight within time span
  • Being overweight: Patients with most recent record of BMI in range 25-29.9
  • Overweight and offered intervention: Patients overweight (above), and Overweight in HP Interventions within the last year
  • Being obese: Patients with most recent record of BMI greater than 29.9
  • Obese and offered intervention: Patients obese (above) and Obese in HP Interventions within the last year
  • Alcohol recorded: Patients with any alcohol record
  • High alcohol: Patients with most recent record of alcohol above 21 units/week for men and 14 units/week for women
  • Managed for alcohol: Patients with high alcohol (above) and High alcohol in HP Interventions within the last year
  • FH CVA/IHD: Any Family History record and a READ Code of 12C.., 1224., 1225., 1226., 12C2., 12C3., 12C4., 12C5., 12C7.,
  • With FH of CVA/IHD: Any Family History record and a READ code of 12C.., 12C2., 12C3., 12C4., 12C5., 12C7.
  • Offered HP for FH: Patients with FH CVA/IHD (above) who have any Health Promotion CVA/IHD record in report range.

Summary for Asthma Management Report

  • Target Population: Aged between 15 and 74 years on report date
  • With asthma: Patients on Asthma Register.
  • On prophylactic medicine: Patients with asthma on Therapy issues or acute prescriptions for bronchodilators (BNF code 3.1), corticosteroids (BNF 3.2), or prophylaxis of asthma (BNF 3.3)
  • PFR recorded: Patients with asthma and a current peak flow record - Asthma Management form, Tabbed Forms
  • Asthma hosp. admits: Patients with asthma and a record of an asthmatic-related hospital admission in Referrals, or Asthma Last Attack - hospitalised on Asthma Management form

Summary for Diabetes Management Report

  • Target Population: Aged between 15 and 74 years on report date
  • With diabetes: Patients on Diabetic Register.
  • Insulin dependent: Patients with diabetes with Therapy of insulin (BNF 6.11)
  • Not insulin dependent: Patients with diabetes and not insulin dependent (see above), ie no insulin record in report range.
  • Annual check - in practice, Annual check - in hospital, Annual check - shared program: On Diabetes Annual Check, Chronic Disease Registers under Type of Program (practice, hospital, shared); also entered on Diabetes Management (tabbed form)
