Background to the Unplanned Admissions DES 2015/16

Practices are required to implement a proactive case management for all patients on the Unplanned Admissions register. The Personalised Care Plan Manager Template is designed to be used collaboratively with the patient and their carer (if applicable). If the patient consents, the plan can be shared with the multi-disciplinary team and other relevant parties as its aim is to make sure the patient's care is managed consistently.

Any new patients coming onto the register in year should have their personalised care plans created and agreed within a reasonable timeframe, but no later than one month after their entry onto the register.
