What is Shared Care?
Shared Care, is how the NHS describes the partnerships between different clinical and administration- services working together in caring for patients, for example:

Groups of GP Practices, within a geographic area, designating hubs that any of their staff can work in and any of their patients can book appointments for, regardless of the clinical systems they use in their practice.

Groups of GP Practices using the same physiotherapy clinic and booking appointments directly for that service.
