Data Entry in Vision 3
To record an immunisation in Vision 3:
- From Consultation Manager , select the patient and start a consultation if required.
- Select Add - Immunisations.
- The Immunisation - Add screen displays:
- Complete as follows:
- Date Given - Defaults to today's date, update as required.
- Given by - Defaults to the staff member or GP signed on for the current session, update as required.
- Private - Defaults to unticked, tick if you are recording a private immunisation.
- READ Term for Immunisation - Inactive until you select Type of Immunisation, it then updates with the clinical term and description relevant to your selection in Type of Immunisation.
- Show All - Defaults to unticked, so only current immunisations display in Type of Immunisation. Tick to display all immunisation options, including historical options in the Type of Immunisation list.
- Type of Immunisation - Select from the list of immunisations.Note - If you cannot find the type of immunisation you require, select either Other Single vaccination or Other Combined vaccination.
- Stage - Depending on the immunisation, select from the available list as appropriate, for example, 1st, 2nd or Booster.
- Status - Defaults to Given, select from the available list as required.Remember - If it is the case, it is important to record that a patient has refused an immunisation.

Due Date - Automatically completes based on the Type of Immunisation and Stage you select, update as appropriate. In Status, if you select Advised, remove the due date. A message displays on saving an immunisation with no due date "It is usual to enter a due date for [immunisation] 2nd. Do you wish to continue. Yes/No ", select Yes to continue.
- Method - Defaults to Intramuscular, select from the available list if required.
- In Practice - Defaults to In this Practice, select from the available list if required.
- Reason - Defaults to Routine Measure.
- Batch Number - Enter the Batch Number from the vaccine if required.Important - Batch Number should only contain the batch number. If the input is over 30 characters or contains a quote (“) character, a pipe (|), a backslash (\), or a carriage return/line feed the data could be rejected causing anomalies between the data entered and the data extracted.
- Site - Select from the list available.
- Notes - Select to enter free text comments if required, for example, a batch expiry date.
- Select OK to save and close.
Note - If you are adding an immunisation before the due date of an existing immunisation of the same kind, a warning displays.
