- Congestive Heart Failure
- Hypertension
- Age (Over 75 years)
- Diabetes Mellitus
- Strokes or TIA
- Vascular Disease
- Age (65 to 74 years)
- Sex Category
The CHA2DS2-VASc risk score is calculated using the following data:

Accessing the CHADS2-VASc - AF Stroke Risk Calculator
- From Consultation Manager, select Vision+ from the floating toolbar and select Calculators - Cardiovascular - CHA2DS2-VASc.
- The CHADS2-VASc - AF Stroke Risk Calculator displays:
- The Clinical Term, Score and Event Date automatically display at the base of the screen.
- Select OK to record the clinical term and score or select OK & Attach to record both the clinical term and the attachment:

The appropriate clinical term saves to the patient's record:
- To view an attachment, from patient's record, right click on the attachment entry and select View Attachment. The attachment displays as a PDF:
