Diabetes and Health Promotion

On the Chronic Disease Register - Diabetes screen, click on if you want to record a Health Promotion intervention, which might be a leaflet, advice, invitation to a Clinic.

If a patient has a diagnosis of diabetes, then their records are relevant for the Health Promotion Banding Report if they fall within the target age of 15-74. For specific Health Promotion information on diabetes, see

Records from Therapy are included for patients on insulin, biguanides, sulphonylureas. Patients on the Diabetes Chronic Disease Register, and Diabetes Annual Check register are included. Any hospital admissions should be entered in Referrals, for a diabetic condition.
