Vaccination Claims Scotland

This Excel report compiles the number of specific vaccinations, to help practices in Scotland complete the 2022 - 23 Quarterly Vaccinations Claims.

This includes the following vaccinations:

• Anthrax
• Cholera
• D.T.P.
• Hep A
• HEP A+B
• MMR
• Meningitis
• Poliomyelitis
• Rabies
• Typhoid
• Pertussis
• Pneumococcal
• Shingles

When the report opens it defaults to the date of the last quarter, this can be updated if required.

When compiled the claim form is automatically completed within Excel.

You can update the numbers on the claim form or delete any vaccines on the actual sheets which automatically updates the claim form.

View the README sheet for information on the colour coding within the report.

Once you are happy with the data on the Claim Form sheet, return to the README sheet and select either:

• Save Claim Form as Excel file for email, or

• Save Claim form as PDF.

Updates

• Updated 15/02/2023 to v3.0:

  • New vaccinations added to the report.

• Updated 07/03/2018 to v2.0 (reference a request from NHS Highland eHealth Department)
  • The patient age is now calculated from a cell on the README sheet, which defaults to the 31st March each year, but may be changed to suit.
  • Shingles Catch-up Cohort has been changed to those Aged 76y
  • 2 additional Shingles mop up cohorts have been included from previous years for patients aged between 71-74 and 77-79
  • Duplicate immunisations are now removed automatically. Unless there are 2 stages as in Intranasal and MenB
