Chronic Disease Management Plans
The progress of a patient with chronic disease can be monitored using Summary Forms or Management Plans specifically designed for clinical management of chronic conditions. These bring together data entered from several sources, such as pathology results and blood pressure reading. One of the entries on chronic disease forms or plans is a Register entry.
Within the patient record, Summary Forms and Management Plans are accessible from:
- Summary - Disease Management - Select as appropriate.
- Navigation pane, right click on the disease required and select Summary Form or Management Plan. Note - If there are no disease registers entries yet, the Management Plan cannot be displayed because the guideline cannot find any relevant data with which to merge. You can, however, add a new entry from the Summary Form.
