Asthma-Add, Asthma Register, Health Promotion
You can enter an initial asthma diagnosis using Add - Medical History. For subsequent asthma consultations, you are advised to use either the Asthma Summary Form or the Asthma Management Plan (see Asthma Management Plan). Both are accessed from the Patient Record, and both group together the entries for asthma management.
Any entry of asthma will prompt the system to ask you if you want to place the patient on the asthma chronic disease register. This places all your asthma patients in one group, regardless of the asthma Read description, and makes recalls easier (see "Chronic Disease Management").
To enter from Medical History:
- First select a patient and start a consultation.
- You can use either Add - Medical History, or Read Term Add.
- Type asthma in the Read description or type #H33 and then:
Either press Enter for H33..00 Asthma, then to view other asthma descriptions one by one, press the down arrow key.
Or press <F3> to reach the Read Dictionary selection screen with asthmatic codes displayed.
- Click on OK. If you started with Read Term Add, you will now reach the History Add screen, which you should complete, e.g. enter any Recall date . Click OK.
- If Automatically select best SDA is unchecked on Consultation - Options - Setup- Patient Record, and if you have added asthma to History-Add, at the Select Clinical Area to Record Data screen, you will be asked if you want to record this structured data under Asthma rather than General History (see Structured Data Area). This is to be recommended so later searches are more specific. You may be given a choice: for example, either Asthma last attack or Asthmadiagnosis. Highlight one. Then click on the button Enter in the SelectedSDA.
- Asthma Register: You will then be asked if you want to add the patient to the asthma register. Answer Yes if so.
- This then shows the Chronic Disease Register - Asthma Add screen which records today's date as the date that the patient is placed on the register. Record a recall if required, or free text notes. Click on OK. See "Chronic Disease Management" which explains this. You can also reach the register screen by selecting Add - Chronic Disease Management - Asthma Register; or click on .
Health Promotion: Once on the Asthma register, the patient's asthma records become relevant for the Health Promotion Banding Report if they fall within the target age of 15-74. The Health Promotion Report is printed from the Search & Report module.
Any asthma records entered in Therapy are included for health promotion purposes for patients on prophylactic medicine. Peak flow rates are recorded on the Asthma Management form (see "Asthma Management Plan").
Any hospital admissions should be entered in Referrals, for an asthmatic condition.
