Identifying the User on a History Entry

When History Add is initialised for the first time, the Clinician box is populated with active clinical staff with the patient's registered GP selected (it is also selected if this GP is inactive - in this case it appears as the only inactive clinician in the list).

• When In Practice is unticked, Clinician is populated with all active/inactive clinical and administration staff. The patient's registered GP remains selected.
• When In Practice is ticked, Clinician is populated with all ACTIVE clinical and administration staff. The registered GP is changed to the user currently logged in.
