From Vision 3 start-up menu, Modules - Populate Read Formulary:
Select File, Formulary Export.
At the Export Read Formulary Details to File screen, give the formulary a filename before the extension of .fmy. It is saved by default to the c:\extract directory.
Click OK.
If successful, you should see a message: The formulary details have been saved successfully. Click OK.
You can then go to the \extract directory and copy the *.fmy file to disk.
