Death Administration

The Summary Form - Death Certificate is the best form to start with. There is no navigation, like a greyed out entry on the Patient Record View for reasons of sensitivity, except through Select Any Form.

  1. From Consultation Manager, select Summary - Death Certificate. The Death Certificate provides a summary of the data entered on the Death Administration and Cause of Death forms.
  2. When the Death Certificate form displays, right mouse click near the top of the form and select Add for Death Administration.
  3. On Death Administration - Add, you can enter the administrative aspects of the Death Certificate. Complete the following:
    • Date of death, offset dates can be used here, for example -2d for two days ago.

    • Date Last Seen Alive, offset dates can be used here, for example -2d for two days ago.

    • Read Code - Defaults to 22J..12 Death, update as required.

    • Notes (and Place of death) - Enter comments as required.

    • Post-mortem information - Select from the list available.

    • Seen After Death - Select from the list available.

    • Employment Related - Tick if appropriate.

    • Date of Completion of Death Certificate - Defaults to today's date, update as required.

    • Clinician Signing Certificate - Defaults to the GP currently signed on, update as required.

  4. Select OK to save.
  5. The Cause of Death screen displays, complete as follows:
    • Category of death - Select from Ia,Ib,Ic,II.
    • Interval from onset to death - Enter as appropriated
    • Cause of death - Select a clinical code as required.
    • Free text - Enter any comments required.
  6. If you want to add more than one category, right mouse click within the Cause of Death screen on the Death Certificate screen and select Add again.
    Note - You can also select Death Administration and Cause of Death separately.

Entering one of the following clinical terms triggers the Death Administration screen:

  • 8HG.. Died in hospital

  • 22J.. O/E - dead, including 22J1.-22JZ.

  • 94... Death administration, including 941..-94Z..

  • R21.. [D]Sudden death, cause unknown, including R210.-R21z.

  • R2...12 [D]<mortality, cause unsure

Select Summary Form - Clinical Entities, scroll down to Death Administration, which has two options, Death Administration and Cause of Death.

Note -You can request a Deduction from the practice register of a patient who has died. This then triggers your TP to send a Deduction Transaction to you, removing the patient. This is done within Registration - Action.
