Register permanent patients with no medical card
To register patients with no medical card:
Patients registering on the permanent list without a medical card could be:
- immigrants
- just arrived from abroad
- gone abroad for longer than three months but was previously registered with an NHS doctor
- private patients now wanting to join the NHS
- an NHS patient transferring to the practice but who has lost their FP4 medical card
- ex-services without a card
- First, check the patient is not already on the computer (Check first that patient is not already on register).
- Select New Patient .
- Follow the sequence in Add a new patient - Existing or new family and complete Registration - Personal Details (initial). In Registration Status, patients will be Applied if going on the permanent list.
- At the Registration - Card Type screen, select None of these, ie the patient does not present a medical card (FP4, FP58 or FP13).
- Then answer whether the patient was previously NHS registered:
- Was the patient previously registered under the NHS (Y/N)?
- Answer Yes if the patient was previously registered with the NHS. Click on OK to display the Registration - Transfer Type screen (see Registration - Transfer Type screen).
- Answer No for patients that are registering without a medical card who were not previously registered with the NHS. Then follow the sequence in Patients not previously registered with NHS.
If the patient is registering without a medical card, but was previously registered within the NHS, at the Registration - Transfer Type screen, choose from:
- been within the medical cover of the Armed Services (see Ex-services without a medical card).
- just come from abroad (see Patient just come from abroad, and immigrants)
- been a private patient (see Private patients)
- noneof these (see Patients who are not ex-services, not private, or not from abroad).
If the patient is registering as a permanent patient but has no medical card, and was previously registered within the NHS, at the Registration - Transfer Type screen, choose from just come from abroad, been a private patient, or none of these.
- If just come from abroad- you will be prompted for the Date Left UK and the Date Entered UK. Both dates are essential so the period of time out of the country can be calculated.
- If more than three months, and the patient was previously registered with an NHS GP, he/she will have been removed from the practice list while away. They will now be regarded as an Acceptance Type of Immigrant and have to re-register.
- If less than three months, the patient will be a Transfer-In. Enter GP Notes (optional free text), Place of birth (mandatory). Click on OK for Registration - Personal Details.
If a previously private patient is now registering as a permanent patient within the NHS but has no medical card, enter None of these at the Registration - Medical Card type screen. Answer whether the patient was previous registered with the NHS. If Yes, then at the Registration - Transfer Type screen, choose:
Been a private patient - This patient will be regarded as a 1st Acceptance - enter GP Notes in free text (optional) and the Place of birth (mandatory if no NHS No. entered). Click on OK for Registration - Personal Details).
If the patient is registering without a medical card, enter None of these at the Registration - Medical Card type screen. Answer whether the patient was previous registered with the NHS. If Yes, then at the Registration - Transfer Type screen, choose:
- None of these - Click on OK, and the patient will be regarded as a Transfer-In. Enter GP Notes (optional free text), Place of birth (mandatory as no NHS No. entered), at least one line of the Previous Address, Previous GP name, and Previous Agency (HA/MB/CSA). Click on OK for Registration - Personal Details.
If at the Registration - Medical Card type screen, you have selected None of these, (ie the patient has no FP4, FP58 or FP13), the next question is:
- Was the patient previously registered with the NHS?
- If you answer No, you are then asked:
- Was the patient’s place of birth anywhere within the UK, or outside the UK?
- If Anywhere within the UK, when OK is clicked, the Acceptance Type - 1st Acceptance screen displays. Enter the GP Notes in free text (optional) and the Place of birth (mandatory as no NHS No. entered). Click on OK for Registration Details screen.
- If Outside the UK, enter the Date left UK (this is optional and can be left blank) and you must enter the Date Entered UK. Click on OK to display the Acceptance Type - Immigrant screen. Enter GP Notes (optional free text), Place of birth (mandatory as no NHS No. entered), Previous Address (optional), Previous GP name (optional), and Previous Agency (optional). Click on OK for Click on OK for Registration - Personal Details.
Continue with the registration entries until you are ready to save - see OK to save when adding new patient.
