Hypertension and Chronic Disease Register

Some entries of hypertension will prompt the system to ask you if you want to place the patient on the hypertension chronic disease register. This places all your hypertensive patients in one group, regardless of the Read description, and makes recalls easier (see Chronic Disease Management). Entering the patient on the Hypertension Register will qualify the record for Health Promotion.

To make an initial diagnosis of hypertension:

  1. Select Add - Medical History or Read Term Add.
  2. Type in #G20 or #G24 or hypert and press Enter, or F3 to reach the full Read dictionary, and select an appropriate hypertension term.
  3. On Read Term Add, click OK once a suitable Read term has been selected, to reach the History Add screen.
  4. Click OK to accept the history entry.
  5. You will be asked if you want to place the patient on the Hypertension register. Answer Yes.
  6. At the Chronic Disease Register - Hypertension - Add screen, click on the Health Promotion icon to record an intervention. Click on the Recall or Notes icons as required. Click OK to accept the entry.

    To add your patient directly to the HypertensionChronic Disease Register, and if required, record Health Promotion interventions:

  7. First select a patient and start a consultation.
  8. Select Add - Chronic Disease Management (Alt-A, N).
  9. Select Hypertension Register.
  10. Confirm the details on the Chronic DiseaseRegister - Hypertension - Add screen: today's day is the default for the date Placed on Register. The Read term defaults to G2 Hypertensivedisease.
  11. Click on the Health Promotion icon to record an intervention.
  12. Click on the Recall or Notes icons as required.
  13. Click OK.
