Summary Forms from Select Data Entry Form

  1. Use one of these ways to access Summary Forms:
    • Click on . Then drill down either All Summary Forms or All Tabbed Forms. To add an entry, point to the required field, eg, Blood pressure, right click and select Add.
    • Add Data - Select Data Entry Form displays all forms under the headings Clinical Data, All Summary Forms, All Tabbed Forms. Similar to CM PRV Summary Forms Icon.
    • Right click on a line (eg Examination Findings) of the navigation pane of the Patient Record and select Summary Form.
    • Summary - Select Summary Form to display the same as the above.
    • From the navigation pane, right click on one of the following lines and select Summary, Data Grid or Management Plan:
    • Examination Findings/Physiological Measurements, Other Findings, Test Results
    • Management - Lifestyle,
    • Management - Routine Checks:Health New Registration ,
    • Child Health Surveillance,
    • Maternity,
    • Female Health Check and Male Health Check (see Health Checks Health Checks)
    • Elderly Assessment
    • or Management - Disease Management: Asthma,
    • Diabetes (see Diabetes Summary Form), CV/BP, Epilepsy (see Epilepsy Data Entries)
  2. If you use one of the menu options, Add Data - Select Data Entry Form, or Summary - Select Summary Form, you will find that the Clinical Management Forms are in three categories: Clinical Data, Summary Forms and Tabbed Forms.
    • Clinical Data are separate data entries, such as, for example, Pregnancy Dates, Risk Factors etc (entries from the Maternity Care form).
    • Summary Forms group clinical data with a relevant theme, such as Pregnancy Start for Maternity Care.
    • Tabbed Forms are a group of summary forms under one category, eg Maternity Care is a tabbed form with several sections for data entry - eg, Pregnancy Start, Antenatal, Investigations, Outcome, Infant Details, Postnatal. On the Maternity Care form, the Investigations section includes all the usual maternity tests on one screen.
    • The icons at the top of the screen are (from left to right): Close Form, Help, Pop-Up Help, Open Form.
  3. Double click on the yellow crossed folder to display further options for either Clinical Data, Summary Forms or Tabbed Forms. This opens the folder and displays any further menu options. If you double click on the open yellow folder again, this closes up the menu options.
  4. Click on the title of the required form and click on the open icon ; or double click on the title of a form. If a form has no folder CM PRV Yellow Paper Icon or CM PRV Blue Paper Icon, then clicking to open this will go directly to the form. If there is folder, however, then there are further menu options, before reaching the form.
