Chronic Disease Management Plans
The progress of a patient with chronic disease can be monitored using Summary Forms or Management Plans specifically designed for clinical management of chronic conditions. These bring together data entered from several sources, such as pathology results, blood pressure reading and so on. One of the entries on chronic disease forms or plans is a Register entry.
Summary Forms and Management Plans are accessible from:
- Summary - Disease Management - Asthma Management Plan, Asthma Summary Form, Diabetes Management Plan, Diabetes Summary Form, CV or Hypertension Management Plan, CV or Hypertension Summary Form, Epilepsy Data Entries
- From the navigation pane (Medical Records Overview column) of the Patient Record - right click on a line, e.g. asthma, diabetes, epilepsy, and select Summary Form or Management Plan, . Note that if there are no disease registers entries yet, the Management Plan cannot be displayed because the guideline will not find any relevant data with which to merge. You can, however, add a new entry from the Summary Form.
