GMS Contract 2017/18 - England

From 1 July 2017, practices in England should use the Vision+ Electronic Frailty Index tools to identify patients aged 65 and over who are living with moderate and severe frailty. For those patients identified as living with severe frailty, and where clinically appropriate you need to:

  • Medication review - Conduct an annual medication review
  • Falls - Discuss whether the patient has fallen in the last 12 months and provide any other clinically relevant interventions
  • Summary Care Record (SCR) - Where a patient does not already have an enriched SCR, you should seek consent from the patient to activate the enhanced SCR. See Summary Care Record England.

You are required to code the above interventions so that data can be collected on the following:

  • Patients with a diagnosis of moderate frailty - Clinical term 2Jd1 - Moderate frailty
  • Patients with severe frailty - Clinical term 2Jd2 - Severe frailty who have:
  • An annual medication review
  • A fall in the preceding twelve months
  • Provided explicit consent to activate their enriched SCR
