Death Administration

The Summary Form - Death Certificate is the best form to start with. There is no navigation (like a greyed out entry on the Patient Record View for reasons of sensitivity) except through Select Any Form.

  1. Select Summary Menu – Death Certificate. The Death Certificate provides a summary of the data entered on the Death Administration and Cause of Death forms.
  2. When the Death Certificate form displays, right mouse click (near the top of the form) and select Add for Death Administration.
  3. On Death Administration –Add, you can enter the administrative aspects of the Death Certificate. There are fields to record:

    Date of death which can be recorded using the abbreviated date format T (today), eg T-1d (yesterday); and Date Last Seen Alive.

    The READ code defaults to 22J..12 Death.

    Place of death is free text.

    Post-morteminformation and Seen After Death both have picklists, and there is a box to tick if Employment Related.

    Date of Completion of Death Certificate defaults to today's date. Clinician Signing Certificate defaults to the GP currently signed on.

  4. When you click OK on completion, it will display the Cause of Death form, where Category of Death can be selected (Ia,Ib,Ic,II), a Cause of Death Read code selected and free text entered in Intervalfrom onset of death. There is a large free text window
  5. If you want to add more than one category, then right mouse within the Cause of death window on the Death Certificate screen and Add again.Note you can also select Death Administration and Cause of Death separately:

    Entry of one of the following Read codes leads to the Death Administration screen:

    8HG.. Died in hospital (now included in Death Admin SDA)

    22J.. O/E - dead, including 22J1.-22JZ.

    94... Death administration, including 941..-94Z..

    R21.. [D]Sudden death, cause unknown, including R210.-R21z.

    R2...12 [D]<mortality, cause unsure

    Or select Summary Form , Clinical Entities, then scroll down to Death Administration, which has two options: Death Administration and Cause of Death.

    Note - If you are linked to your TP (Health Authority) for Registration, you can request a Deduction from the practice registerof a patient who has died. This then triggers the TP to send a Deduction Transaction to you, removing the patient. This is done within Registration – Action.
