Ways to Add and View Data

Before adding clinical data in Consultation Manager, you must first select a patient and ensure you have an open consultation. Editing or listing data does not require a consultation to be started first.

Data is mostly entered and stored within Consultation Manager using terms from the Read dictionary.

There are many different ways to add data:

  • Type a keyword in the Read term add box at the bottom on the Journal/Problems screen.
  • Click on the Add menu, or press Alt-A for the Add menu, then press the underlined letter - eg Alt-A-B for Blood Pressure-Add.
  • Use Vision+ Alerts which display when QOF or information from practice protocols is missing on the patient record.
  • Use Vision + Templates to access QOF and other clinical templates which provide step by step protocols and links to other screens.
  • Use the Vision Apps to quickly add, view or edit a range of basic summary screens and simple data entry forms.
  • Or from the left-hand navigation pane on the Patient Record, right click on a line and select one of the Management Plans which list suitable entries in a category, eg Asthma Management Plan.
  • Or list previous data entries for a patient and use the right mouse menu, selecting Add (see Add entries).
  • Or click and drag using the Drag and Drop for some specific entries, including recall and referrals.
  • Or select the form you want, eg Blood Pressure-Add, make the entries and click OK. Or you can just make an entry in Read Term - Add and let the system place your entry in the correct structured data area.

If you make an entry such as a blood pressure reading on Blood Pressure-Add, the data displays automatically on any other screen which has the same BP field, such as the Examination Findings or Well Person Clinic Management Plans or summary forms. Some forms go directly into add mode, while others, such as List or Medical Records Overview menu, allow you to view previous entries first. To add from these screens, use the right mouse for options, and select Add.

All entries made to the system are listed chronologically on the Patient Record Journal View.
