History

Asthma Register – When the patient was placed on the Asthma Chronic Disease Register. This is usually the original entry of the diagnosis as H33 Asthma.
Asthma Diagnosed – Defaults to H33.. Asthma, but scroll down in Read Term and choose a more relevant entry Also select from picklist for How asthma was diagnosed (e.g. Night cough, Chest symptoms etc).
Current Asthma Status – 663..11 Asthma Monitoring – in Current status of asthma, select from <None> Observation only, Continuous treatment, Resolved, Intermittent treatment.
Asthma Risk Factors – select from <None>, Not at risk, Previous history of severe attack, On 3 or more drugs, Night symptoms, Recent hospital admission, Other reason.
