Five Shot Questionnaire

The Five Shot calculator is a short alcohol questionnaire designed to be used to recognise alcoholism in its early stages. The five-shot test is composed of two questions from the AUDIT test and three questions from the CAGE test and is intended to be a shortened version of the AUDIT test.

To access the calculator:

1. From Consultation Manager, select a patient and click on the Vision+ icon on the floating toolbar and select Calculators.

2. Select Lifestyle and choose Five Shot.

3. Complete the questionnaire by selecting an option under each heading. On completion of all the questions the Read code 38Df - Five-shot questionnaire on heavy drinking displays along with the patient's score.

4. Click OK to save the data to the patient's record or OK and Attach to also record the questionnaire.

   Note - The Five-shot Read code is saved under Test Results - Scoring Test Results.
