Health Check Forms You Can Use
There are a number of Health Check forms for well patients of different age and sex:
- Well Person Clinic – for the Female Health Check or Male Health Check
- New Registration Examination - for newly registered patients
- Child Health Surveillance for children under five years
- Maternity
- Elderly Assessment for over 65 patients
- Lifestyle
These can be selected in the number of ways:
- From the Summary Menu – Routine Checks (Alt-S, U), then New Registration, Child Health, Maternity, Female Health Check, Male Health Check, Elderly.
- From the Medical Records Column of the Patient Record, and right mouse click on either Well Person Clinic, Lifestyle, Elderly, CHS, Maternity, New Registration etc.; then select either Summary Form or Management Plan. If any of these lines is coloured dark red, then there are already existing entries; left mouse click on this line to list these chronologically under the Filtered tab.
- Click on
and select Clinical Entities - Well Person ClinicorLifestyle. This lists the separate Add screens for all asthma entries; or select Tabbed Forms - either Female Health Check or Male Health Check.Note - Similar entries of BP, weight, height etc, can be made on Cardiovascular Risk Assessment, Physiological Measurements and Prevention Details screens.
