Managing your Patients using the CKD Manager Template

The CKD Manager template is a simple to use screen that is designed to provide pertinent clinical information at a glance. It is interactive and only allows you to add information that is relevant to the patient. Using the data entry tools you can view historical data or add new data. You can also graph numerical information.

Key Points

  • Adding Data - To add data to the CKD Manager screen you can either use the Data Entry Tools at the end of each line or the quick clinical term add option.

  • Viewing Data - The latest recorded entry displays on the CKD Manager screen. To view historical data click the red Previous Entries icon. A blue Previous entry icon indicates that the patient does not have any previous records.
  • Comments - Freetext comments can be entered or, if applicable, selected from a drop-down list.
  • Save and Close - All entries made to the screen should be saved by selecting Save and Close . If you do not save and close your data will be lost.
  • Results - The top part of the screen shows the patient's latest test results and investigations. This information automatically populates from the clinical system. To view historical data click the red Previous Entries icon. A blue Previous entry icon indicates that the patient does not have any previous records.
  • Laboratory Tests Needed - If you do not use electronic test requesting you can record test requests in the Laboratory Tests Needed section of the screen. If you click the freetext comments box you can select Retest U/E's and the frequency, for example 1-2, 3 or 6 weeks.
  • Needs Repeat Urine Sample - From this section of the screen you can record the fact that the patient needs a repeat urine sample. If you select the freetext comments box you can select the sample type, for example Urine dip, ACR or PCR.
  • NICE CKD Staging - This area of the screen allows you to quickly clinical term the patient's CKD stage. The clinical terms only become available if the patient's CKD stage requires changing or if you want to place a newly diagnosed patient on the CKD Register.
  • Patient Aware of Diagnosis - This section of the screen allows you to document if the patient is aware of their diagnosis. Because there is not a specific clinical term for CKD diagnosis awareness you can select CKD from the freetext comments box.
  • CKD Exceptions - If applicable to a patient, this region of the screen is where you can record the CKD QOF/QAIF Exception clinical term and optionally enter a freetext comments.
