Viewing the Yellow Card Report

From the Yellow card screen, to view the report you have created, select Preview . To change the page, use the arrows in the top right corner .

The following information displays on the report:

Page 1

Patient Details - Anonymised details from the patient record

Suspected Drug(s)/Vaccine(s) - Drug details from the DrugAllergy and Intolerance screen

 

Page 2

Suspected Reaction(s) - Read Term for Reaction from the Drug Allergy and Intolerance screen

Medically significant; please give details: - Clinical Narrative; Please give details: from the Drug Allergy and Intolerance screen

Other Drug(s) - Automatically populated from the patient record

 

Page 4

Medical History - Automatically populated from the patient record

Additional Relevant Information - Free text added in the Yellow Card sections, Free Text (Results & Procedures) and Additional Information (Medical History)

Reporter Details - Your name, practice, and contact details populated from the Practice and Staff records held in Vision - Management Tools - Control Panel - File Maintenance

Clinician (if not the reporter) - If this Yellow Card is being submitted by an administrator, the details of the clinician selected in Drug Allergy andIntolerance are completed
