What are Chronic Disease Registers?

Patients with chronic diseases can be placed on Chronic Disease Registers (see Chronic Disease Register - add, list or remove a patient). This has the advantage of grouping patients under one register, although their Read descriptions for the condition might vary. Searching for patients for recall is therefore easier. The categories for the chronic disease registers are:

Placing a patient on the Asthma, Diabetes, CHD, Hypertension or Stroke/TIA register will include their records for the Health Promotion Banding Reports, if they fall within the age group of the target population for Health Promotion (15-74). Do this by recording a Health Promotion (see ).

To list any existing register entries from the navigation pane of the Patient Record, click on the Disease Registers line to display a chronological list under the Filtered tab.

The thyroid disease register was removed in DLM 186 and Thyroid disease records placed in Superseded Records at the bottom of the navigation pane (see Thyroid disease).
