Yellow Card Reporting

The Yellow Card Scheme collects and monitors information on suspected safety concerns or incidents involving medicines and medical devices.

There is a dedicated Yellow Card site for the reporting of new and emerging side effects and medical device incidents in COVID-19 treatments, this includes the reporting of side effects for medicines taken by patients to manage long-term or pre-existing conditions.

Important - Please report any new and emerging side effects and medical device incidents in COVID-19 treatments here.

It is important you do not use the integrated Yellow Card functionality within Vision 3 for the reporting of the side effects and medical device incidents in COVID-19 treatments.

Recording in Vision 3

Optionally, you can record the Yellow Card report in the patient’s record using the following:

  • Read Term for Characteristic – 9G4..11 Yellow card drug react notif
  • Comment - Medication/Appliance name – COVID-19
